Гонорея — это заболевание вызванное грамм негативным подвижным диплококком, который может проникать в межклеточное пространство с образованием микроколоний.
Классификация гонореи у женщин:
- Гонорея нижних отделов моче половых путей.
- Гонорея нижних отделов моче половых путей с осложнением.
- Гонорея верхних отделов моче половых путей.
Клинический выделяют:
- Острая.
- Под острая.
- Торпидная.
- Хроническая.
Клиническая картина гонореи у женщин
В большинстве случаев заболевание протекает торпидно или бессимптомно. Симптомы зависят от локализации повреждения и похожи с клинической картиной эндоцервицита, аднексита, пельвиоперитонита. У женщин даже при острым течении заболевания не возникает болевых чувств. Характерным отличаем есть возникновение большого количества участков воспаления в местах локализации цилиндрического эпителия, а именно мочевик, цервикальный канал, бартолиновые железы. Внешние половые органы и вагина у женщин репродуктивного возраста, как правило не поражаются.
Диагностика
Диагноз гонорея ставится только при лабораторном определении гонококков. К современным методам диагностики гонореи относятся ПЦР (полимеразная цепная реакция), ЛЦР (лигазная цепная реакция). Материал для исследования берут из трех мест, цервикальный канал, мочеточник, вагина, желательно исследование проводить сразу после окончания месячных. Из остальных мест мазок берут при наличии надобности (прямая кишка, глотка).
Лечение
Лечение гонореи проводят антибиотикотерапией. Группами выбора являются цефалоспорины, фторхинолоны, аминогликозиды. Иногда в редких случаях используют пенициллин, современная медицина пытается не использовать его из-за высокой резистентности штаммов гонококка к нему. Беременных и детей у которых обнаружен гонококк лечат цефалоспоринами и макролидами, остальные антибактериальные препараты беременным противопоказаны. К вспомогательной терапии относятся иммуностимуляторы, ферменты, физиотерапия. Повторное исследование на наличие штаммов гонококка делают через 7-10 дней после окончания лечения и через 1 мес. окончательное.
